Chronic Disease Clinics
Special clinics for patients suffering from Chronic diseases such as Heart Disease, Hypertension, COPD and Diabetes are held at both surgeries. Patients suffering from chronic diseases will be offered an annual review with a specialist nurse. Appointments are usually arranged during your birthday month.

We offer Antenatal clinics at both sites led by our Midwife Fleur Charlton.
Our nurses are trained to provide a number of services. Please call nurse reception if you require a service that is not listed here.
Family planning clinic
You can discuss your contraceptive requirements with any of the doctors or Nurse Practitioners. For routine ‘pill checks’ you can also book an appointment with any of our practice nurses.
Nurse Practitioner Sarah Gallagher and Dr Sally Davies are trained to fit contraceptive devices.

Tests can be performed by our practice nurses. Women should have a test every three years from the age of 25. You will be notified by letter when your smear test is due. If, after three notification letters and a doctor’s letter, a patient fails to attend for a smear, the responsibility for the smear test will rest with that patient.

Our nurses both at the Portland Practice and at the Hatherley Branch Surgery are trained to provide travel advice and vaccinations. Please submit a Travel Form using eConsult 6-8 weeks before your visit.
If you are travelling within the next 2 weeks we suggest you contact a private travel health clinic to ensure you get your vaccines in time for your trip.

We aim to provide a report or copies of records within the following timescales:
- Medical report completed by the GP – 4 weeks
- Copies of records – 8 weeks
